Adipocytes are active endocrine organs. They produce leptin (satiety signal), adiponectin (insulin sensitivity) and inflammatory messengers like TNF-α. Removing a local fat depot also influences the hormonal microclimate.
Today we distinguish white fat (energy storage) and brown fat (thermogenesis). Subcutaneous deposits are mostly white fat — exactly what liposuction addresses. Cell number is relatively constant from late adolescence. Diets change cell size, not number. That's the biological reason why local deposits are so stubborn.
Tumescent solution is more than anaesthesia. It contains lidocaine, adrenaline, buffered saline. Adrenaline constricts blood vessels — massively reducing blood loss (from formerly 40-50% hematocrit to under 5% today).
This technique (Klein method, 1980s) revolutionised liposuction. What Dr. Fetai modernised: 2:1 to 3:1 volume ratio (tumescence to fat), tuned to body zone. So the tissue stays pain-free for exactly the OP duration + 4-6 hours after.
Skin is not a passive cloth. After volume reduction, fibroblasts activate new collagen production. This response takes 3-6 months — hence the long final-result phase.
Key retraction factors: age, skin elasticity, hormonal status, genetics, smoking. Younger patients with good elasticity retract spontaneously. With mature skin we combine biostimulators (Sculptra, Radiesse) — the iatrogenic wound healing response is intensified.
The body has a biological weight set-point regulated via the hypothalamus. After diets it fights actively back to the previous weight — the notorious yo-yo effect.
Liposuction does not change this set-point. It reduces local fat-cell count. Meaning: with weight gain new fat distributes more evenly. From endocrinology's perspective, liposuction is body sculpting, not obesity therapy. Whoever understands this has realistic expectations.
Body image is not just optics. It's a neural representation — and it reacts differently to surgical change than expected.
In the parietal cortex there's a 'map' of your body. This map updates after changes — about 6-8 weeks after liposuction. Until then the brain still 'feels' the old volume. That explains the often-described 'phantom feeling' early on.
